A blank four-question rating template for structured session check-ins and progress reviews.

Where This Tool Helps

This template provides space for four coach- or client-defined questions and an optional five-point response scale. A number has meaning only within the wording and endpoints the user defines.

Common uses include session-opening check-ins (rate your current confidence on these four dimensions), session-closing reviews (rate your clarity on four areas we worked on), or between-session tracking of specific variables the client is monitoring.

The format is intentionally minimal. The value comes from the questions you put into it, not from the template itself.

How to Use This Template

  1. Before distributing to a client, fill in the four questions in the question fields. Questions should be specific, not general ("Rate your confidence in delivering the presentation Friday" rather than "Rate your confidence").
  2. The 1–5 scale reads left-to-right from low to high. Consider labeling the endpoints (e.g., 1 = Not at all, 5 = Completely) for each question where the meaning of the scale might be ambiguous.
  3. Clients circle or mark their chosen rating. One rating per question.
  4. Review the four ratings together at the session opening or close. Compare ratings only if the questions and scale meanings are sufficiently consistent, and do not infer progress or cause from a change.
  5. Keep or discard previous copies according to the client's privacy choice and applicable record-handling practice.

Choice and Support Boundary

Optional coaching reflection, not therapy, diagnosis, medical care, a validated assessment, or business, financial, tax, accounting, investment, legal, employment, or relationship advice. Skip, adapt, or stop. Do not infer a cause, condition, trait, motive, truth, score, risk, priority, or required response from an answer, blank, label, difficulty, repetition, or estimate. Preserve privacy, consent, access, culture, power, context, and uncertainty; use qualified or emergency support for unsafe, severe, or out-of-scope concerns.
