Five Dimensions of Self-Care

Self-care is not a reward for finishing your work - it is the foundation that makes sustained, effective work possible. Coaches regularly observe that clients who are depleted in one or more dimensions of self-care struggle to make progress on their professional goals. The problem is rarely a lack of motivation. It is a system running on reserves it no longer has.

This assessment covers five dimensions: psychological and emotional, social, physical, spiritual, and professional. Completing it honestly produces a picture of where you are functioning well and where the gaps are. That picture is more useful than any generic advice about self-improvement.

How to Use This Assessment

  1. Rate each item honestly using the scale below. Use 1, 2, 3, or ★ for each behavior.
  2. Work through all five sections before analyzing your results.
  3. Notice which sections have mostly 1s and 2s - these are your current priority areas.
  4. Bring your completed assessment to your next coaching session for discussion.
  5. Return to this assessment every 90 days to track changes over time.
