What This Survey Measures

Most people enter coaching knowing something feels off — without being precise about where. This survey maps current satisfaction levels across four domains: work, relationships, health, and personal growth. The ratings alone are useful. What's more useful is the pattern they create together and how honestly the self-liking scale reflects the overall picture.

The coaching goals checklist at the end is often where the real priorities surface. Many clients discover that the domain they listed last — almost as an afterthought — is where the most meaningful work is waiting.

The readiness question is not a formality. A score of 6 and a score of 9 lead to very different first conversations with a coach, and knowing the gap between “what I want to change” and “how ready I am to change it” is itself the first piece of self-knowledge the process requires.

The sequence below is designed to move from observation to intention — completing the ratings before the open questions, so your written responses have something concrete to react to rather than starting from a blank slate.

How to Use This Survey

  1. Complete the four satisfaction ratings first, without overthinking any individual score. Your first instinct is usually more accurate than your considered one.
  2. Rate yourself on the self-liking scale separately. Notice whether it correlates with your satisfaction scores or diverges from them.
  3. Answer the three open questions in order. The first two are descriptive; the third is where you move from assessment to intention.
  4. In the coaching goals checklist, check any domain you want to work on — not just the ones where you scored lowest. Low scores don't always indicate readiness; high scores don't rule out growth.
  5. Complete the readiness scale last. If your score surprises you, note what you think it's reflecting.
