Goal and Practice Context Map

Goal Setting Tools

Optional client-owned prompts; use, adapt, reorder, or skip based on context.

Using This Optional Tool

This page offers optional prompts to record a goal, why it may matter, current practices, supports, constraints, and unknowns without treating habits as causes or requirements. The fields are prompts, not findings, requirements, or a complete model of the situation.

Use the client's own words and distinguish observations from interpretations and unknowns. Any field may be renamed, reordered, left blank, revisited, or discarded.

How to Use Goal and Practice Context Map

  1. Choose one relevant situation, example, or question, or skip the page.
  2. Record observed information separately from interpretations and unknowns.
  3. Use any number of fields in any order; no answer, blank, rating, difficulty, or repetition proves a cause, trait, priority, risk, or outcome.
  4. Review or share only what the client chooses and keep any next step optional.

Choice and Support Boundary

Optional coaching reflection - not therapy, diagnosis, medical care, financial, tax, accounting, investment, legal, or employment advice, and not a validated assessment. Skip, adapt, or stop. Do not infer causes, conditions, traits, motives, truth, risk, priority, or required responses. Preserve privacy, consent, access, context, and uncertainty; use qualified or emergency support for unsafe, severe, or out-of-scope concerns.
