Where This Tool Helps

Medication consistency is harder to maintain than it looks on paper. Mornings move fast, routines get interrupted, and when you are already late for a call, a midday dose is easy to skip. The problem is not intention - it is the absence of a system that catches the misses before they accumulate.

This tracker gives you that system. It records what you took, when you took it, and how you felt - so that over time you and your prescriber have actual data instead of a rough impression of how the week went. That data matters: patterns in missed doses, timing inconsistencies, or side effects that only appear on certain days are only visible when they are written down.

One important note: this tracker supports your medication routine, it does not advise it. Decisions about dose, timing, or switching medications belong between you and your prescriber. Bring this log to those conversations - it gives them something concrete to work from.

How to Use This Tracker

  1. Fill in your medication names and dosages at the top before the week begins. If they change mid-week, note it in the Side Effects/Notes column.
  2. Mark each dose as soon as you take it. Not at the end of the day. The checkbox is only useful if it is checked in the moment - "I'm pretty sure I took it" is not the same as a checked box.
  3. Log the time alongside the checkbox. Timing drift - taking morning medication an hour later each day - is invisible without the time column and can explain inconsistent effectiveness.
  4. Use the Side Effects/Notes column for anything notable. A headache, reduced appetite, an unusually focused afternoon, a difficult evening. One or two words per cell.
  5. Review missed doses at the end of the week. Note the total and what was going on those days. Bring the completed log to your next prescriber appointment.
