Where This Tool Helps

The gap between a goal and an outcome is almost always a planning gap, not a motivation gap. An action plan works by making that gap visible and specific. When each element is documented - not just the goal but the steps, the timeline, the milestones, and the resources required - you can see exactly where you are and what the next move is.

The section that most clients underuse is Resources. It asks what you need to accomplish the goal that you do not already have - time, information, skills, relationships, budget. Leaving it blank is common and costly. The goal stalls not because of poor motivation but because a resource requirement was never named.

The steps below move through the plan in the order that produces the most clarity.

How to Use This Worksheet

  1. In the Goals section, write the outcome you are working toward. Be specific enough that you would know when you had achieved it.
  2. In Action Steps, list the specific tasks required in the order they need to happen. If you cannot name the first three steps, the goal is not defined clearly enough yet.
  3. In Time Frame, set a completion date and identify any interim deadlines for the action steps. Assign dates, not just "soon."
  4. In Milestones, name the checkpoints that will tell you the plan is on track. These are observable signs of progress, not just more steps.
  5. In Resources, name what you need that you do not currently have - and who or what can provide it.
  6. In Desired Outcome, write what success looks like in concrete terms. This should be specific enough to serve as a completion criterion.
